If you are interested in being seen in the cleft lip/palate craniofacial clinic, you need to call (405) 271-2866 and ask to talk to the team coordinator.
If you have e-mail capability you can request the information by emailing info@profilesofs.com. You will receive a packet in the mail with the registration information. If you will fill out the information and return it promptly we will then make an appointment for the next available clinic.
